What is IUI?

Intrauterine insemination (IUI) is a common fertility treatment that can bring hope to many couples struggling to conceive. It involves placing a concentrated amount of good-quality sperm directly inside the woman's uterus at the time of ovulation. However, the two-week wait after the procedure can be an emotionally challenging time. If you've recently undergone IUI, here's what you should expect for the next two weeks.

The IUI process

Before IUI

Your fertility specialist will assess the reasons why you are struggling to conceive. Afterwards, they will determine if IUI is the best fertility treatment for you and your partner.

Once the sperm has been placed, most people can immediately return to their everyday activities. However, some women can experience some mild symptoms after IUI treatment, such as mild cramping and spotting for around 2 days. While there are no restrictions on the different types of activities you can do after the procedure, it is best to avoid strenuous activities that make you feel discomfort.

Day 1 to 3

During the first 3 days, it is normal to experience cramping and light spotting, as they are common symptoms of artificial insemination. This should not be confused with implantation bleeding. However, if the discomfort and cramping do not stop after 3 days, please consult your fertility specialist.

Day 6 to 14

This period is close to the implantation window. During this period, you could experience light spotting, also known as implantation bleeding. You may also experience cramping or other types of discomfort. Other symptoms of early pregnancy, indicating the IUI procedure's success, include:

  • Stomach discomfort

  • Constipation

  • Dizziness

  • Nausea

  • Vomiting

  • Breast tenderness

Exhaustion. However, in most cases, these symptoms do not occur until after several days of implantation. It is normal if you don't have these symptoms after day 6; remember to continue monitoring your symptoms day by day until you miss your period.

Day 14 onwards

14 days after your IUI procedure, you will need to visit your doctor for a pregnancy check. High hormone levels are a symptom of a successful IUI treatment. You could also take an at-home pregnancy test, but they are not as accurate as a lab test. However, if the test is negative and you missed your period, you may need another test.

Timing of pregnancy tests after IUI

Generally, fertility doctors recommend waiting about 14 days after the IUI procedure before taking a pregnancy test. If you test too soon after the IUI treatment, you may get a false negative because your hormone levels have not risen enough to be detected. This timeframe allows for:

  1. Fertilisation of the egg

  2. Implantation of the embryo (which takes 6-12 days)

  3. Production of detectable levels of human chorionic gonadotropin (hCG) hormone

By day 20, most pregnancy tests should be accurate. However, it's important to understand that various factors can influence test results.

Possible reasons for a negative test at 20 Days

  • Failed IUI cycle

    • Unfortunately, not all IUI procedures result in pregnancy. Success rates vary depending on individual circumstances.

  • Late implantation

    • In some cases, implantation might occur later than usual, delaying the production of detectable hCG levels.

  • Testing error

    • While rare at 20 days post-IUI, there's a small possibility of a false negative due to testing too early or using an insufficiently sensitive test.

  • Chemical pregnancy

    • A chemical pregnancy indicates that conception occurred, meaning that fertilisation is possible. However, early pregnancy loss, or an early miscarriage, occurs shortly after implantation and often goes undetected unless you have early and sensitive pregnancy tests.

What to do next?

If the IUI procedure is unsuccessful and you've received a negative pregnancy test 20 days after IUI, consider the following steps:

  • Consult your fertility specialist

    • They can provide personalised advice and may recommend a blood test for more accurate results.

  • Wait for your period

    • If your period doesn't arrive within a few days, consider retaking the test or consulting your doctor.

  • Discuss next steps

    • If this IUI cycle was unsuccessful, your fertility specialist can help you plan future treatments or explore other options.

  • Seek support

    • The emotional toll of fertility treatments can be significant. Don't hesitate to seek support from loved ones or professional counsellors.

It's important to have a thorough consultation with your fertility specialist to determine if IUI is suitable for you. Request a consultation with our fertility specialist at the Thomson Fertility Centre for personalised recommendations and tailored advice.

Remember, a negative test doesn't necessarily mean the end of your fertility journey. Many couples succeed after multiple IUI attempts or by exploring other fertility treatments. Oftentimes, it could take around 3 to 6 cycles of artificial insemination before you succeed.

Your fertility specialist could also recommend medications for both you and your partner before the next cycle to boost the success rates of IUI. Stay hopeful and maintain open communication with your healthcare provider throughout the process.

FAQ

Can you get a false negative pregnancy test after IUI?

Testing too soon could produce a result that is “false-negative”. This means that the test finds no sign of pregnancy when, in fact, you really are pregnant. You might get a false-negative result if pregnancy hormones aren't yet at levels that can be measured.

How long does it take for hCG to show up in urine after IUI?

You'll know if you're pregnant approximately two weeks after IUI. It takes about that long for hCG to be detected in blood or urine. Your healthcare provider will let you know if you should return for a blood test to detect pregnancy or if you can use an at-home urine test.

Can you have a negative pregnancy test at 16 days after IUI but no period?

You can confirm pregnancy about 14 days after the IUI procedure. That's when it's best to take a pregnancy test. While you can do an at-home test, the one your doctor does will be more accurate. If your test is negative but you still miss your period, you might need to test again.

How late can a period be after IUI?

Most women can expect their period to arrive approximately 14 days after undergoing an IUI procedure. This 14-day period serves as a reliable guide, much like having a scheduled event marked on a specific date in your calendar.

How to know if IUI failed?

A pregnancy test is the only way to be sure whether the IUI has failed or not. During the two-week wait from the insemination to the pregnancy test, you may experience bleeding or a lack of pregnancy signs, which may indicate an IUI failure.

Can a failed IUI cause an early period?

A failed IUI procedure itself does not typically delay the menstrual cycle. However, various factors associated with undergoing fertility treatments like IUI, such as stress, hormonal medications, and emotional impact, can potentially affect the timing of menstruation.
